At which speed should the electric mixer fitted with the dough hook be set?

First start with the paddle to mix and then switch to the hook. It should never go above medium. Once the dough begins to ball around the hook remove it and finish kneading by hand on a floured board.

Mix on medium-low before the dough clumps up around the dough hook. Once the dough clumps up, reduce the speed to low.
