yeast may be dead, yeast may not have been activated correctly, ambient temperature may be too cold, too much gluten may have developed
If you mean the shop bought puff pastry. Could be too warm when rolled, or ironically oven isn't warm enough. To make it rise. Keep as cool as possible before putting it in a hot oven.
