i baked chicken at 400 degrees for 40 minutes and its slimy inside. i would like to know why?

A couple things come to mind. Was the chicken cut in pieces or whole? Was it cold right from the fridge?
I use a meat thermometer and keep it in the oven longer if needed. Renting a piece of foil over it helps too.
