What am I doing wrong.....Why are my cookies so flat?

I made these with freezing, and without freezing. They came out extremely flat, to the point my husband actually asked what happened to them. They definitely Taste great but... what am I doing wrong ?

Are your baking powder and baking soda relatively new purchases? If they are old, they may be relatively inactive
If they are ok, we need to look for other possible causes.
