I'm thinking it may be because
1) if the questioner doesn't see it come up immediately, he/she will hit the "add" button a second time or
2) if no one answers, the questioner will post the query a second time

This should be appearing less and less now that we're coming 'up to speed.'
And I think sometimes the same person posts by twitter and by a 2nd method.

I was thinking the same as susan g and creamtea. What I have been doing is flagging the repeat questions, saying they are repeat.
