Why is there foam in the water of simmering green beans, bacon and onion? We have done this for years and have never seen this? Pan was rinsed well.

The foam is protein being released into the water. If you've done this before I'm surprised you haven't seen it happen. This process always produces a bit of "scum". It's easily removed but is harmless to consume.
