Bre - you can add them before or after water boils and oats are cooked, but I find before easier and gives better flavor to the oatmeal.

You could cook all except the milk or cream with the oats. Then top with milk or cream. Yes, you could consider them optional  - omit or replace with, for example  another sweetener or fat.
