Chocolate on toffee turned chalky

I made 3 batches and all 3 had issues. The first batch I used a copper clad stainless pot and it started to scorch. The second batch I used a non-stick Calphalon pot and the butter separated. I managed to mix it back in somehow. Then 24 hours later the chocolate turned chalky. Can you tell me why those things happened?
