Why is it grainy when reheated?

The flavor of this is phenomenal but when I reheat it it is grainy, even with whisking. Am I not getting it hot enough before whisking? Or did I do something wrong when making it to begin with? Thank you!

Try adding a little bit of water when you reheat, to re-dissolve the sugar crystals and heat until it dissolves. You might have to do this every time.
