Simmering with or without lid?

Do you cover or leave uncovered for simmering?

Your preference, based on equipment and your judgment.
If the pot is deep and there is little risk of splattering, you could do uncovered.
If the contents nearly fill the pot and/or you want to concentrate the flavor, cover the stew.
Or split the difference...put the cover on slightly askew so it's mostly covered. Then adjust if needed during the cooking.
